The following forms will assist you registering for Dynamic Skating. Unless otherwise noted, these forms should be sent as soon as possible and no later than the first day of camp:
- Medical Release Form (mandatory for Week Long Camp)
- Camper Pick Up Release (required for non-parental pickup)
- Sunscreen/Insect Repellent Form (mandatory for Week Long Camp)
